VACCINATIONS: Dogs boarding at River Valley Animal Hospital must be current on their DHPP, Bordetella (kennel cough) and Rabies vaccinations. Cats boarding at RVAH must be current on their FVRCP and Rabies vaccinations. If you are not a client of RVAH, please provide proof from your treating veterinarian as to the status of your pet’s vaccine and internal parasite history. If your pet is not current on any of the required vaccinations or internal parasite testing, this will be updated by RVAH and your bill adjusted accordingly.

INTERNAL PARASITES: All pets staying at RVAH must have current fecal (stool sample) examination within the past 6 months (one year for cats) indicating that the pet was negative for internal parasites. Pets with fecal negative results more than the time limit listed above will be tested at RVAH and if the pet is found to have any internal parasites, they will be treated accordingly for the parasites at the owners expense.

EXTERNAL PARASITES: All pets will be examined for external parasites. If fleas or ticks are found, your pet will be treated for their own wellbeing as well as for the safety and wellbeing of the other pets at RVAH at the owner’s expense.

ILLNESS/INJURY: Reasonable precaution will be used against injury, escape or death of this pet. In the event of injury or illness, I understand my pet will be treated as deemed best by the attending veterinarian and staff and I assume full responsibility for the treatment expense within reason. In the event of an emergency, RVAH will attempt to contact you and inform you of your pet’s condition ASAP; if you are unreachable your emergency contact listed above will be contacted. RVAH and its employees will not be held liable for any personal injury or property damage or any injury, illness or disease sustained by my pet stemming from their participation in boarding provided reasonable care and precautions are followed.
